If the Level I is being completed on an individual admitting to a nursing facility under PACE, you will need to choose PACE as the payment source when completing the Level I online. For questions or assistance regarding PACE, please refer to the PACE Information Sheet posted under Evaluator Tools.

Attention Colorado Users
Hospice Level I referrals. If the Level I is being completed on a individual admitting to a nursing home under Hospice, you will need to choose Hospice as the payment source when completing the Level I online.
