Do I need to Complete a Level I Screen?

1. A Resident of A Nursing Facility in Colorado This means that the individual resides in a nursing facility or was transferred from a nursing facility directly to a hospital and will return to the same or different nursing home. Exception: If the person was admiiting to the NH under a Convalescent Approval, further approval through Ascend is required. Submit a new Level I pre-admission and resident review form.
- For individual with a known or suspected diagnosis of Major Mental Illness, If patient transferred to medical or psychiatric hospitalization and is returning to same NH, No New Level I is required. There must be documentation of PASRR approval which reflects the client's current status. If not, contact the OBRA Coordinator for a Status Change. Click here to find the Obra Coordinator in your county. If patient transferred to psychiatric hospitalization and is going to a new NH, a status change through Ascend must be preformed.
- If individual has a known or suspected Developmental Disability No New Level I is required, but the Division for Developmental Disabilities (DDD) must approve ALL NH transfers. Contact The Division at 303-866-7143. There must be documentation of PASRR approval which reflects the client's current status. If not, contact the OBRA/PASRR Coordinator for a Status Change. 2. A Resident of an Out of State Nursing Facility
Transferring to a Colorado Medicaid certified nursing facility
Level I is required

3. Admission to NH under HCBS Respite
- HCBS Respite only applies to individuals on home and community based service waiver through Medicaid and is needs NH services for 30 days or less. No Level 1 is required, but the county Medicaid office MUST approve the admission.
- If the patient is needing NH services for more than 30 days, a Level I must be completed prior to admission. Proceed to login.
4. An Applicant to a Colorado-based Nursing Facility
The individual is not a current resident of a nursing facility and is applying to a Colorado-base Medicaid Certified nursing facility.

5. A Waiver Recipient transferring to NF
The individual is participating in a waiver program, such as the Elderly Blind and Disabled (EBD) or other Home and Community Based Service Program (HCBS) and is seeking long term nursing home placement.
Level I is required

6. NF Resident who Requires a Continued Stay Review
This means that the individual's payer is Medicaid, she or he was medically approved for a time limited stay in the nursing facility, and the Medicaid approved time period will soon end.
No New Level I is required. The client's Level I must accurately portray the client's current status and there must be no evidence or suspicion of mental illness, mental retardation, or developmental disability. If mental illness, mental retardation, or developmental disability is known or suspected, contact OBRA/PASRR Coordinator to initiate a status change. 7. Experiencing a Payment Source Change
This means that the individual is a current nursing facility resident and his/her method of payment has changed. [for example, from Medicare to Medicaid or self pay to Medicaid].
No New Level I is required. There must be documentation of PASRR approval which reflects the client?s current status. If not, contact the OBRA/PASRR Coordinator for a Status Change. 8. Expiring PASRR Short Term Stays
This means that, at admission, the individual was believed to have mental illness, mental retardation, or developmental disability and was approved for a short term nursing home stay [such as convalescent care] and the approved time period will soon end.

For individuals who may have MR/DD, the Division for Developmental Disabilities will initiate the status change evaluations.
